Building strength, restoring confidence, and preparing for sport after ACL reconstruction at APEX PWR in Tigard, Oregon
ACL reconstruction recovery is a journey that doesn’t end once you’re able to walk without crutches. In fact, the mid-stage of ACL rehab (4–6 months post-op) is one of the most important phases. It is the bridge between regaining basic mobility and preparing the body for the explosive, dynamic demands of sport.
At APEX PWR in Tigard, Oregon, our team specializes in guiding athletes through this critical phase of ACL rehab, ensuring progress is both safe and performance-focused.

The mid-stage typically begins around 4–5 months post-op, depending on the surgeon’s protocol and patient progress. By this point, swelling has subsided, range of motion has returned, and baseline strength is being rebuilt.
The goals shift from basic function to advanced strength, movement quality, and sport-specific preparation.
“At this stage our focus is building high-level strength, restoring normal movement patterns, and preparing the athlete for more sport-specific demands.”
Key objectives include:
Strengthening the muscles around the knee, especially the quads and hamstrings, is essential. But mid-stage ACL rehab is not just about lifting more weight.
It is about retraining the nervous system. After an ACL injury and surgery, the brain often hesitates to fully trust the reconstructed ligament. Without targeted rehab, athletes may develop faulty movement mechanics, increasing their risk for future injuries.
The mid-stage of ACL rehab is where we can sometimes see the biggest leap in confidence. It is not only about restoring muscle strength, it is about retraining movement so the knee feels reliable again. Athletes need both the physical and mental side of recovery to be ready for sport.
“These drills not only build muscle, but also retrain the brain to trust the knee again after surgery.”
While there are many options for physical therapy in the Tigard, Beaverton, SW Portland, Oregon area, there aren’t any facilities that are equipped with experienced physical therapists, pro-athlete level resources, testing technology, equipment and overall capacity.
At APEX PWR, we integrate VALD Performance technology and force plate testing to objectively measure progress during mid-stage rehab. This data shows athletes (and parents, coaches, or teams) exactly how strength, balance, and movement symmetry are improving.
Instead of guessing when it is safe to jog, jump, or cut, we use hard data to guide the timeline. This precision is what helps our athletes return stronger and safer than before.
🎥 Watch the video below to see one of our athletes at five months post-op ACL reconstruction. The focus is on advanced strength drills, movement pattern restoration, and early sport-specific preparation. These drills build confidence, restore power, and prepare the athlete for the final stage of return-to-sport rehab.
The same principles apply across ages. Adults recovering from ACL reconstruction move through the same mid-stage priorities: rebuilding quad and hamstring strength, restoring movement quality, and retraining the knee to be trusted under load.
Start with a physical therapy evaluation. Our sports PT team takes athletes from early rehab all the way through sport-specific return-to-sport training, in Tigard and across the Portland metro.
Skipping steps or rushing this stage can lead to setbacks and even re-injury. Studies show athletes who return to sport too soon after ACL surgery are at significantly higher risk of tearing the same knee or the opposite one.
By dedicating the right amount of time and focus to mid-stage rehab, athletes:

Dr. Dustin Zink, DPT, is a sports physical therapist at APEX PWR who specializes in guiding athletes through the mid and late stages of ACL rehab. Mid-stage ACL rehab typically runs from about 4 to 6 months post-op. Swelling has settled and range of motion has returned, so the focus shifts to advanced strength, movement quality, neuromuscular control, and early sport-specific preparation like running, cutting, and agility drills.
Full sport-specific readiness after ACL reconstruction commonly takes 9 to 12 months or more. Timeline alone is not the deciding factor. Return-to-sport is determined by restored strength, movement quality, and objective testing, not just the calendar.
Rushing or skipping the mid-stage raises re-injury risk. Athletes who return too soon are at significantly higher risk of tearing the same or opposite knee. Mid-stage builds the strength foundation and neuromuscular control that make return-to-sport safe.
