Creatine for Women: What the Research Actually Shows

For most of the time creatine has been studied, it has been studied in men. That is not a conspiracy, it is just how a lot of sports science got built: young male college athletes were the convenient sample. The downstream effect was that creatine got culturally filed under bodybuilders and football players, and a generation of women skipped the single most researched performance supplement on the shelf.

That is changing, and it is worth being glad about. Female-specific creatine reviews and meta-analyses are now being published, which means women finally get answers drawn from women.

So when a new female-specific creatine meta-analysis started circulating on social media a couple of weeks ago, we went and read it. Here is what we found, including the part that is less exciting than the graphic suggested.

The Study Everyone Is Posting About

The paper is real. It is a systematic review and multilevel meta-analysis by Lin, Wenfeng and Hong, accepted by Frontiers in Nutrition on 19 August 2026, covering creatine's effects on performance, physiological outcomes and body composition in females who exercise or train.

The version going around says it found creatine "consistently improves strength, power, and high intensity performance in women."

Here is what it actually reports. The authors screened 13,244 records and included 34 studies in the review, of which 21 studies and 424 analyzable participants contributed to the meta-analysis itself.

An effect size of 0.23 to 0.30 is small. One of the four domains came back statistically non-significant. And the authors rate their own certainty as low to very low, which is a formal grading that means future research could easily change the estimate.

The authors' own conclusion: "Available female-specific evidence suggests possible small effects on selected strength, power, high-intensity performance and lean-mass-related measures. Substantial heterogeneity, methodological limitations and low or very-low certainty prevent firm sport-specific or outcome-specific conclusions."

"Possible small effects" and "consistently improves" are not the same sentence. Two other details worth knowing: strength, power and high-intensity performance were pooled together into one performance domain rather than each being demonstrated separately, and the paper never claims to be the first female-specific creatine meta-analysis. It cites the review it builds on in its own background section.

Why we are telling you the unflattering version. We are a testing facility. We put people on force plates and in DEXA scanners and hand them numbers with error bars on them. If we oversold a supplement on a graphic we did not read, you would have every reason to distrust the next number we give you.

The good news is that the honest version still supports taking creatine. It just supports it for different reasons than the ones in the caption.

The Female-Specific Evidence That Is Actually Strong

If you want the best current case for creatine in women, it is not the study in the graphic. It is the work in postmenopausal and older women, which happens to be the population with the most to gain.

Postmenopausal women, 7 trials, 608 participants

A 2026 systematic review and meta-analysis in the Journal of the International Society of Sports Nutrition pooled 7 randomized controlled trials in postmenopausal women, median duration 38 weeks, mean age around 62.

Lean mass increased by 0.37 kg (95% CI 0.05 to 0.69). Leg-press one-rep max increased by 7.5 kg (95% CI 2.2 to 12.8). Benefits were evident when creatine of at least 5 g per day was combined with resistance training. Bone density did not change overall, and adverse events were mild and similar to placebo. (Naddafha et al., JISSN, 2026, PMID 42141930.)

Note what that finding is conditional on. Not creatine alone. Creatine plus resistance training.

Older females, and why 24 weeks is the number to remember

A 2021 meta-analysis in Nutrients looked at 10 randomized trials of creatine combined with resistance training in older females, 211 participants total. Overall, upper-body strength improved significantly. Lower-body strength and muscle mass did not.

Then they split the trials by duration. In trials lasting 24 weeks or longer, both upper-body and lower-body strength improved. (Dos Santos et al., Nutrients, 2021, PMID 34836013.)

That is the most practically useful finding in this entire article. If you take creatine for six weeks, get impatient and quit, you have run the version of the experiment that does not work.

The finding that cuts against the hype

We would rather you hear this from us than find it later. A 2022 analysis in Nutrition pooled 35 studies and 1,192 participants looking at creatine and lean body mass, then broke results out by sex.

Lean body mass: males +1.46 kg (95% CI 0.47 to 2.46). Females +0.29 kg, not statistically significant (95% CI -0.43 to 1.01). (Delpino et al., Nutrition, 2022, PMID 35986981.)

And a 2025 systematic review of active females found that across the performance literature, most studies showed no improvement compared to placebo (Tam et al., Nutrients, PMID 39861368).

So the fair summary is this. Creatine in women is a real but modest effect that shows up most reliably in strength rather than size, in longer trials rather than short ones, and always on top of resistance training. That is a genuinely useful thing to know, and it is a much better basis for a decision than a caption.

What This Means for How You Train

Every positive result above required lifting. The creatine did not build anything on its own, it made the training slightly more productive. Which means if you are choosing between buying creatine and starting a program, start the program.

The practical version for most women we work with is straightforward. Get on a progressive resistance training program you will still be running in six months, take 5 g of creatine a day without thinking about it, and measure something objective so you know whether it is working.
